RN Clinical Case Manager – Home Health

Full‑time position with Nurses at Home, a small intimate agency that values clinicians’ input.

We are looking for a full‑time RN clinical case manager with at least 1 year of homecare experience and the ability to proficiently complete OASIS.

• Manage and coordinate patient care plans for home health clients.
• Complete and submit accurate OASIS assessments.
• Advocate for client needs and ensure high quality, compliant care.
• Collaborate with interdisciplinary team and owners to improve care processes.

• Registered Nurse (RN) license (required).
• Minimum 2 years of RN experience (preferred).
• At least 1 year of home care experience, familiarity with OASIS.
• Strong communication and interpersonal skills.
• Proficiency in English, both written and spoken.

• Monday to Friday.
• Weekend on‑call rotation.

  On the road.

Mandated vaccination, PPE usage.

Bachelor’s degree preferred.
