Hartford Health Care Medical Group is seeking a dedicated nurse leader to coordinate the individualized care of patients across the continuum of care services. You will serve as patient advocate, educator, and a primary liaison between the patient and all other services involved in care.
Requirements include five years' nursing experience, two years of supervisory/management and patient/staff education experience, and an RN license.
