We are seeking a dedicated and experienced Assessment Nurse to join our team at Home Well Care Services IL250 in Glendale Heights, IL. As an independent contractor, you will play a crucial role in assessing the needs of our clients and developing care plans to ensure their well-being. This is a per visit pay job

• Conduct comprehensive assessments of clients' physical, emotional, and mental health
• Develop individualized care plans based on assessment findings
• Coordinate with other healthcare professionals to ensure holistic care for clients
• Provide education and support to clients and their families
• Document assessment findings and care plans accurately and timely

• Valid RN license in the state of Illinois
• Minimum of 2 years of experience in home health or a similar setting
• Strong communication and interpersonal skills
• Ability to work independently and collaboratively within a team
• Compassionate and empathetic demeanor
