Host Home Provider
Res Care Community Living – an affiliate of Bright Spring Health Services with five decades of experience in supporting adults with intellectual, developmental, or cognitive disabilities.
Role SummaryA Host Home Provider is an independent contractor who opens their home to an adult with a developmental disability. Responsibilities include personal care, hygiene, meal planning and cooking, medication management, recreation, transportation, shopping, and other supports as outlined in the Individual’s Service Plan.
Responsibilities- Receive the Individual into the Provider’s home and provide appropriate daily care, activity, and full-time supervision in accordance with applicable laws and the Individual’s Service Plan.
- Prevent any acts of disrespect, abuse, neglect, or exploitation, and immediately report such acts to the company and applicable agencies.
- Document the Individual’s progress and unusual events; submit required reports to meet legal obligations.
- Arrange routine or emergency medical and dental care consistent with the Individual’s Service Plan.
- Provide transportation for service appointments and community activities as needed.
- Notify the company’s Clinical Supervisor or appropriate on‑call staff immediately in the event of serious illness, injury, or death.
- Communicate promptly and regularly with the company about matters affecting care or placement.
- Maintain confidential records and logs regarding the Individual at all times.
- Complete daily notes and grids thoroughly.
- Attend ISP, IEP, or BSP meetings monthly or as scheduled.
- Be on time for all meetings and appointments.
- Provide at least 24‑hour notice when cancelling or rescheduling visits.
- Help train Relief providers/staff before the first day of relief.
- Submit required training documentation for compliance.
- Report changes (visitors, family, medical, behavioral) to QP.
- Review new MARS and Physician Orders, update information, and sign off daily.
- Complete required labs and follow up on results.
- Attend annual visits (physical, dental, vision) and obtain clarifications from doctors.
- Limited travel may be required.
- Perform all other duties as assigned.
- High School Diploma or GED equivalent.
- Training in the CAP Core Competencies.
- Valid driver’s license, reliable vehicle, and good driving record.
- Excellent verbal and written communication skills.
- Comfortable assisting residents with hygiene and medical routines.
- Outstanding organizational and people skills.
USD $48,000.00 per year.
