Job Title
LPN II - Fast Track
LocationWilliam Newton Hospital
SchedulePart‑time position – three shifts per week:
- Mon–Sat 11:30 am to 9:30 pm
- Sun 12:00 pm to 8:00 pm
Provides direct and indirect patient care in a Fast Track or Urgent Care setting. At times will assist RN in the Emergency Department setting. Provides care that reflects initiative, flexibility and responsibility indicative of professional expectation with a minimum of supervision. Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system.
Communicates with Fast Track/ER providers about changes in patient’s status, symptomatology and results of diagnostic studies. Is able to respond quickly and accurately to changes in condition or response to treatment.
- Provides direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure optimal patient care.
- Performs a head‑to‑toe assessment on all patients and reassessments as per policy, including pediatric, adolescent, geriatric, and general patient populations.
- Assesses and reassesses pain and pain management techniques; educates the patient and family regarding pain management.
- Demonstrates knowledge of EKG/ECG and can identify dysrhythmias.
- Assists the Practitioner with office procedures; maintains medical and surgical asepsis.
- Maintains medical equipment; disinfects and sterilizes instruments.
- Answers telephone calls in a professional manner; refers patient problems to House supervisor, ED Manager or other appropriate person.
- Maintains accurate and continued clinical documentation including patient histories, conditions, treatments, medication reconciliation, responses, and assessment of changes.
- Treats patients and their families with respect and dignity; identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs.
- Functions as liaison between administration, patients, physicians, and other healthcare providers.
- Demonstrates knowledge of the principles of growth and development over the life span; provides age‑appropriate care.
- Interacts professionally with patient and family and involves them in the formation of the plan of care.
- Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; includes family as appropriate.
- Has knowledge of medications and their correct administration based on age and clinical condition; follows the seven medication rights and reduces the potential for medication errors.
- Demonstrates knowledge of lab draw procedures and other specimen collection.
- Maintains established Fast Track/ER policies and procedures, objectives, performance improvement program, safety, environmental and infection control standards.
- Plans and provides nursing care with physicians’ orders.
- Maintains a safe, comfortable, and therapeutic environment for patients and families in accordance with office/clinic standards.
- Ensures an adequate stock of supplies and proper functioning of equipment.
- Assists in cost containment through appropriate ordering and conservation of supplies and equipment.
- Cooperates with other personnel to achieve Fast Track/ED objectives and maintains good employee relations.
- Demonstrates knowledge of healthcare plans, procedural costs.
- Assists physicians/mid‑level providers with procedures in the Fast Track/ED.
- Participates in educational programs, current literature, in‑service meetings and workshops; demonstrates responsibility for own professional practice and ongoing education.
- Shows flexibility, organization and functions under stressful situations.
- Demonstrates awareness of current state, federal and local laws governing the delivery of care.
- Performs other related duties as assigned or requested.
- Adheres to dress code; appearance is neat and clean.
- Completes annual education requirements.
- Maintains regulatory requirements, including all state, federal and CMS regulations.
- Maintains patient confidentiality at all times.
- Reports to work on time and as scheduled; completes work within designated time.
