Patient Coordinator

Position: Trillium Health Partners Patient Coordinator Role
Become a vital part of Trillium Health Partners as a Patient Administrative Coordinator. This role is key to improving patient experiences through robust administrative support and effective information management.

As a Patient Administrative Coordinator, you will facilitate important communication within the unit and manage the flow of patient information. Your interactions with patients and families, alongside handling appointment scheduling, will contribute significantly to the quality of care provided. Familiarity with administration tools like Epic is highly valued.

Key Responsibilities:

• Coordinate patient inquiries and manage information flow

• Prepare patient charts and assist in registration processes

• Efficiently schedule appointments for patients

• Support communication efforts for care coordination

• Maintain up-to-date patient records and perform data entry

Requirements:

• High school diploma is required; additional education is a plus

• 1-2 years of relevant administrative experience

• Customer service experience in healthcare is beneficial

• Strong organizational and time-management competencies

• Proficiency in Epic and other computer applications
