We are currently seeking Registered Nurses to join our Community Nursing teams across the County Durham and Darlington localities. The teams provide a service between the hours of 8am and 9pm 7 days a week on a duty roster. The role involves providing holistic nursing care to adult patients in their own home or residential care setting. Successful applicants will be required to hold a first level nursing registration with the NMC, additional training specific to the role can be provided.

The Community nursing team provide both planned and reactive care including Urgent Community Response.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

From April 9th 2025 - Healthcare assistants - You must have been employed for a minimum of two years as a HCA within the NHS for the Trust to consider sponsorship (we will only consider sponsorship if you meet this criteria and have a current visa expiring within 3 months).
Job responsibilitiesThe successful candidate will provide a variety of nursing care to patients, including wound care, administration of medication, palliative care, management of bowel and catheter care. The role involves assessment of patient care needs, care planning, delivery of care and review. The successful candidate will be required to work independently as part of the wider Nursing team reporting back to senior staff on duty.
PersonSpecification Qualifications
- 1st Level Registration with the NMC (Adult)
- Knowledge of issues in community nursing
- Knowledge of Intermediate Care
- Knowledge of key policy drivers for health and social care
- 1st Level Registration with the NMC (Adult)
- Experience of audit/research
- Experience of clinical supervision
- Evidence of recent on-going nursing development
- Good communication and interpersonal skills
- Evidence of Organisational skills
- Evidence of Team working
- Community specific skill set
- Able to travel across Trust sites to meet the requirements of the post
