Utilization Facilitator; N4
Position Overview
Under the general direction of the Utilization Manager and Facility Program Director(s) – Medicine/Family Medicine and in close collaboration with the Facility Vice President responsible for Utilization, Physician Bed Manager(s), and Clinical Manager(s), the Utilization Facilitator is responsible for facilitating and coordinating the movement of patients through the admission, transfer, and discharge processes primarily within the Adult Medicine/Family Medicine Program, ER Program, and other clinical programs as required.
The role also involves liaising between internal and external programs and agencies on matters related to patient movement, participating in the development of guidelines and educational strategies related to patient movement, and contributing to decision‑making that results in an effective admission process and optimal utilization of programme and facility resources.
- Minimum 4 years recent nursing experience.
- Recent acute clinical experience preferred.
- Graduate from a recognized programme in Nursing.
- Bachelor of Nursing degree required.
- Educational preparation in utilisation an asset.
- Maintain and provide proof of active registration with the College of Registered Nurses of Manitoba (CRNM).
- Active participation in professional associations (e.g. Association of Regulated Nurses of Manitoba (ARNM)) preferred.
- Cardiopulmonary Resuscitation (CPR) training requirements for this position shall be in accordance with the employer policy.
- Scope of practice as documented in the Regulated Health Professions Act (RHPA).
- Demonstrated ability in utilizing computerized applications, such as Microsoft Word and Excel.
- Demonstrated ability to function in a pivotal role as facilitator in the patient admission/discharge/transfer process between patient, family, physician, Clinical Manager, members of the Interdisciplinary Care Team, other programmes, community resources, etc.
- Demonstrated ability to initiate and maintain effective resource contacts.
- Demonstrated ability to work with all levels of staff (direct care, interdisciplinary team members and management).
- Demonstrated ability to analyse and problem‑solve.
- Excellent interpersonal and communication skills with co‑workers, management, patients and families.
- Preference will be given to those applicants competent in an Indigenous language and/or knowledge in Indigenous customs, traditions and values.
This term position may end earlier as outlined in your collective agreement.
