This position may be filled by either a Social Worker or Occupational Therapist. Salary will be determined based on the successful candidate's professional designation and corresponding collective agreement. The posted wage range reflects the full range across both applicable classifications.

The Outreach Clinician – Occupational Therapist (OT)/Social Worker (SW), will work as a member of a regional specialty interprofessional mental health team. The team strives to improve the clinical outcomes for individuals with serious mental illness in South‑Eastern Ontario by engaging in the provision of clinical services and community capacity enhancement activities. The team configures their services to provide collaborative care, mental health services, and resources.

As a member of the specialized interprofessional team, the Outreach Clinician - OT/SW is responsible for providing expertise to contribute to recovery‑oriented individualized service plans and to augment and develop resources regionally.

Skills and Qualifications

Required:

Master’s Degree in Occupational Therapy or Social Work required
Current and unrestricted registration with the College of Occupational Therapists of Ontario or College of Social Workers and Social Service Workers
Evidence of advanced education and training in Cognitive Behavioural Therapy, Interpersonal Therapy, and Dialectical Behaviour Therapy (as relevant to the population served), certification preferred
Two years’ of progressively responsible, current clinical experience providing evidence based assessment and intervention services including experience in facilitating evidence based groups (CBT, IPT, DBT) as appropriate for the population served, to individuals with serious mental illness within a Recovery framework
Experience consulting with mental health workers and community mental health agencies, in the design and implementation of evidence based practices for individuals with serious mental illnesses
Experience and confidence working within a broad range of non‑traditional, community settings and with a variety of service providers
Experience in implementing capacity enhancement and knowledge exchange activities
Experience and knowledge of concurrent disorders as relevant to the position
Experience working with transitional aged youth as relevant to the position

Position Responsibilities:

Receives referrals from internal and external sources
Screens each inquiry for urgency of need and suitability of services in order to ensure that individuals receive to the most appropriate service in a timely manner
Develops and maintains a therapeutic relationship with service recipients which include assertively reaching out to them in their natural environment
Involves the service recipient in the scope and focus of the assessment in accordance with their expressed needs and desires
Provides recovery focused assessment services as an Occupational Therapist/Social Worker, using a standardized process and provides access to interprofessional assessment services in order to enhance the client’s and the referral source’s understanding of the presenting problem(s) and possible solution(s)
Provides recommendations for the development of recovery oriented treatment plans to both the referral source and the service recipient in order to enhance the referral source’s ability to provide services to their service recipients
Provides profession specific, evidence‑based and time‑limited interventions, including, but not limited to Cognitive Behavioural Therapy, Interpersonal Therapy, Dialectical Behaviour Therapy, Action Over Inertia, and Changeways, in individual and group formats as relevant to the client population served. Interventions shall be directed to enhance service recipients’ readiness, capacity to make positive changes, ability to achieve their desired goals, ability to manage their symptoms, and function in daily life
