To qualify, candidates must have a Bachelor’s in Social Work or Nursing and a minimum of three years’ experience in a healthcare setting. Understanding of Indigenous cultures and priorities is vital for fostering relationships and facilitating patient transitions. Engage with communities to enhance health services effectively.
Key Responsibilities:
• Oversee patient transfers between renal units and facilities
• Cultivate relationships with Indigenous organizations
• Promote culturally appropriate patient care practices
• Actively participate in team communications
• Travel to various health sites for community support
Requirements:
• Bachelor’s degree in Social Work or Nursing required
• Minimum three years of experience in health services
• Current Manitoba Class 5 Driver’s License required
• Familiarity with Indigenous health protocols essential
• Strong interpersonal and communication skills required
