Reporting to the CKD Program, you will be responsible for conducting Best Possible Medication History (BPMH) primarily in CKD clinics and for in-centre hemodialysis patients. Your technical expertise will support medication distribution, inventory control, and collaboration with Clinical Pharmacists ensuring safety and quality patient care.
Key Responsibilities:
• Distribute medications to patient care areas accurately
• Conduct Best Possible Medication History with patients
• Direct patient and staff queries appropriately
• Issue and deliver controlled medications safely
• Maintain medication inventory records compliant with regulations
Requirements:
• Registration with Ontario College of Pharmacists as a Pharmacy Technician
• Diploma from an accredited Pharmacy Technician program
• Proven attendance and compliance with safety policies
• Empathetic approach in patient interactions
• Experience in BPMH and familiarity with pharmacy automation preferred
