In this dynamic position, you'll collaborate with Clinical Case Managers to enhance participants' life skills, ensuring they lead safe and fulfilling lives. Full-time and part-time options are available to fit your schedule.
Key Responsibilities:
• Document participant progress and support activities
• Write detailed incident reports with the clinical team
• Foster goal-setting and life skills planning
• Ensure comprehensive awareness of participant health
• Accomplish additional duties as assigned
Requirements:
• Two years of direct support experience needed
• Standard First Aid and CPR certification required
• Ensure clear background checks are up to date
• A Human Services educational background is advantageous
• Possess excellent communication and crisis resolution abilities
