BridgeCare Clinic Health Worker

The ideal candidate will have two years of experience in community settings, particularly engaging with populations facing various challenges such as mental health issues. You will work collaboratively with a dedicated team while facilitating outreach programs and support networks.

• Help clients access resources for personal growth

• Implement community development projects effectively

• Engage collaboratively within an interdisciplinary team

• Promote collaboration and communication among stakeholders

• Collect and manage outreach service data and reports

Requirements:

• Required two years of experience in community work

• Must possess a valid driver’s license and vehicle

• Understanding of community resources and health determinants

• Strong communication skills for client interactions

• Outreach training completion is preferred
