In this role, you will work with a dedicated healthcare team to ensure seamless transitions for discharged patients. Responsibilities include conducting health screenings, engaging in community outreach, and providing medication counseling. You will employ your clinical skills to manage medications effectively, ensuring all patients receive personalized and attentive care.
Key Responsibilities:
• Collaborate with physicians and healthcare teams for drug therapy success
• Conduct health screenings and community outreach activities
• Oversee immunization clinics and patient medication reviews
• Provide personalized patient counseling and advice
• Ensure precise preparation and distribution of medications
Requirements:
• Licensed with the College of Pharmacists of Manitoba
• Outstanding communication and interpersonal abilities
• Strong problem-solving skills required
• Injection Certification mandatory
• Experience with Kroll Pharmacy software is a plus
