In this important position within the Priority Home Service, you will engage with hospital teams and community care professionals to devise and implement care plans that address the needs of clients. You will ensure a seamless transition of services and monitor ongoing delivery until clients are ready for their next steps.
Key Responsibilities:
• Collaborate with healthcare teams for holistic service planning
• Handle referrals and service request initiation
• Monitor service delivery for clients until transition
• Perform client assessments to tailor services
• Document all interactions accurately and timely
Requirements:
• Relevant degree in health or human services must be completed
• Experience with community health services is an advantage
• Excellent communication and organizational skills required
• Must possess a valid driver's license and vehicle
• Registered with a Manitoba licensing body in good standing
Utilize your clinical judgment and coordination skills to enhance the wellbeing of clients within the Winnipeg Health Region.
