St.Amant is seeking a Transition Coordinator, Pediatrics to support patient flow and collaboration between Health and Transition Services, Children’s Hospital and community-based care. You will work under the IPAC & CCP Manager to advance family-centered care.
The role requires two years of pediatric clinical experience and familiarity with IDD support. Strong communication, education and discharge planning skills are essential.
