RN Case Manager

Position: Full-Time RN Case Manager
Bayshore Medical Personnel is seeking a full-time RN Case Manager to enhance care coordination for patients in Winnipeg, MB. This role focuses on managing complex health cases within a multidisciplinary team.

As an RN Case Manager, you will utilize your skills to conduct thorough assessments, develop personalized care plans, and ensure ongoing client monitoring. Candidates should have a minimum of three years of nursing experience, along with relevant case management expertise to meet the diverse health needs of our clients.

Key Responsibilities:

• Perform detailed case management assessments for clients

• Develop customized care plans based on health needs

• Monitor clients' progress and recommend modifications

• Collaborate with multidisciplinary primary health care teams

• Provide educational support to clients and families

Requirements:

• Baccalaureate Degree in Nursing

• Minimum three years of clinical nursing experience

• Current provincial nursing license

• BLS or CPR/AED Level C certification

• Willingness to work daytime hours, Monday to Friday
