Position Summary The Licensed Practical Nurse provides direct nursing services to individuals with physical and intellectual disabilities that focus on health promotion, restoration, maintenance, and prevention of illness and disability and palliative care. The Nurse is responsible for providing total patient care but may delegate some aspects of care to other health care team members in a staff mix model (e.g., Resident Assistants).
In addition to providing direct practical nursing services, the Nurse functions as a key member of the professional health care team and is responsible for providing day‑to‑day instruction and feedback to the Resident Assistants assigned to their care team.
Duties and Responsibilities Client Care Management:
Provide comprehensive direct nursing services to assigned people supported, in keeping with the scope of Nursing Service’s mandate and the full scope of clinical practice identified by the CLPNM. Assessment duties include: Assess people supported using a variety of resources and focusing on biological, emotional, socio‑cultural, spiritual and quality of life factors.
Use knowledge from practical nursing and data from other members of the health care team to analyze and interpret the data necessary to identify potential and actual health needs/problems.
Perform ongoing assessment to identify changes in person supported health status.
Planning duties include: Develop individualized care plan in collaboration with other health team members.
Include the person supported and their family, as possible, in developing the individual, focused nursing/health plan of care.
Supports individual health and growth and development by building on personal, family and community strengths and established support systems.
Supervises the implementation of the individualized plan of health and personal care.
Treatment duties include: Organize work for the efficient delivery of person supported care.
Provide direct nursing care to assigned group of individuals.
Provide health information to person supported, family and legal decision makers in an appropriate and timely manner.
Facilitate a least restrictive environment and promote awareness of people supported rights and a person‑centred approach to care.
Delegate care appropriately based on assessment of people supported needs and competence/role of care providers and monitor outcomes of delegated care.
Communicate health care concerns to the physician and coordinate supported individuals to be seen at the regularly scheduled medical rounds.
Demonstrate competence in the performance of technical skills (e.g., administration of medications, treatments) and use appropriate technology to provide safe effective nursing care.
Promotes and supports a living and learning setting that focuses on the individual’s abilities and promotes physical, mental, social, emotional and spiritual health.
Work with all members of the health care team in implementing the individual person supported care plan.
Evaluation duties include: Objectively evaluate the effectiveness of nursing interventions by comparing actual outcomes to anticipated outcomes.
Participate in coordinating the appropriate links with physicians, nurses and other health care disciplines at Health & Transition Services to ensure continuity of care.
Work with people supported, families, legal decision makers and other members of the health care team to reevaluate and modify interventions according to changes in health status and assessment findings.
Advise the Resident Care Coordinator of any significant changes in supported person’s health status.
Maintain complete and accurate documentation of all relevant health care concerns, observations and interventions in the health care record using the approved method of documentation.
Qualifications Educational:
Graduate of an approved LPN educational program.
Professional: Holds current active practicing membership with the CLPNM.
Active CPR/Code Blue competence.
Practises within the professional, ethical, legal and legislative parameters of the Licensed Practical Nursing Act, CLPNM Standards of Practice and Canadian Nurses Association Code of…
