Registered Nurse in Integrated Care Team

Key Responsibilities:

• Provide high-quality nursing care to community patients

• Collaborate with healthcare team members for patient-focused care

• Ensure nursing practices are current for successful outcomes

• Assess and respond to changing patient needs

• Travel to patients’ homes within the region

Requirements:

• Minimum two years nursing experience required

• Registered with College of Nurses of Ontario in good standing

• Proficient in English, additional languages an asset

• Valid Ontario driver’s license and reliable vehicle

• Experience with infusion therapy and wound management
