This full-time or part-time role requires an IIWCC certificate or NSWOC designation and involves delivering holistic, patient-focused nursing care. You will assess and create care plans for patients with wound and ostomy concerns, ensuring that you respond to their needs effectively and proactively. The position includes traveling to patients’ homes within a specified area.
Key Responsibilities- Perform comprehensive assessments of patient needs
- Collaboratively create tailored care plans for patients
- Provide secure, holistic nursing services
- Anticipate and adapt to patient care changes
- Travel to various home locations in the community
- Must have NSWOC or IIWCC certification
- Recorded good standing with the College of Nurses of Ontario
- Minimum of two years nursing experience preferred
- Valid Ontario driver’s license and reliable vehicle
- Basic proficiency in Microsoft Office applications
