This full time, permanent role focuses on delivering comprehensive care across the lifespan. You will collaborate with multidisciplinary teams while incorporating traditional and Western medicine practices. Your commitment to culturally safe care will make an impact in urban Aboriginal populations and surrounding First Nations communities.
Key Responsibilities- Deliver primary care services, including assessments and treatments
- Manage chronic conditions collaboratively with clients and families
- Prescribe medications and order diagnostic tests
- Provide trauma‑informed mental health support
- Incorporate traditional healing methods with Elders and healers
- Master's degree in Nursing or accredited NP program
- Registered with the College of Nurses of Ontario
- Minimum 2 years of primary care or Indigenous health experience
- Knowledge of health issues affecting Indigenous communities
- Clear Criminal Record Check and valid Ontario driver's license
