Location: Morden
Become a vital part of the Grenfell Health Centre team as a full-time Nurse Practitioner. Provide crucial primary health care services to the community with a focus on assessment and treatment.
This full-time role involves working in primary care teams under the Primary Health Care Manager’s direction. You will assess, plan, implement, and evaluate patient care both independently and collaboratively with physicians. Responsibilities include health promotion, emergency services, supportive counseling, and community needs assessments.
Key Responsibilities:
• Provide primary health care assessments to patients
• Collaborate with physicians to support complex cases
• Conduct health promotion and disease prevention services
• Initiate community needs assessments as needed
• Engage in evaluative projects with health professionals
Requirements:
• One year in community needs assessment/development
• Three years as an RN in critical or primary care
• Master of Nursing - Nurse Practitioner degree
• Advanced Cardiac Life Support certification
• Pediatric Advanced Life Support certification
