About the RoleThe Nurse Field Case Manager (FCM) is responsible for a collaborative process of ongoing assessment, planning, facilitation, service coordination, evaluation, and advocacy to meet the needs of patients, physicians and clients through flawless customer service. The FCM delivers quality and cost‑effective treatment outcomes by working directly with the reimbursement team to secure drug coverage and support physicians.
They manage a busy caseload in their territory, building working relationships, collaborating, communicating effectively and paying attention to detail. They support physicians in managing patient access to therapy, provide service in person or remotely, and adhere to SDM Specialty Health Network SOPs, policies, and the College of Nurses standards.
Key Responsibilities- Assist HCPs in completing program enrollment forms and educating them on requirements.
- Gather patient enrollment information (physician signature, contact info, product info).
- Provide reimbursement assistance to patients and physicians.
- Support physicians with payer documentation and investigate reimbursement options.
- Determine patient eligibility for reimbursement and assist at all levels.
- Help physicians populate Special Authorization (SA) forms by reviewing charts, previous therapies, and test results.
- Coordinate and follow up on SA submissions with payers.
- Support physicians in developing appeal letters.
- Act as a resource for physicians regarding reimbursement criteria.
- Assist with annual prescription renewal and SA renewal processes.
- Identify obstacles to coverage, take action, and find alternative channels.
- Maintain patient files in the program database.
- Educate HCPs and patients on program services using approved materials.
- Review program components and value‑add offerings.
- Provide feedback to manufacturers on quality, training and service improvements.
- Collaborate with sales reps and associates according to guidelines.
- Attend scheduled educational events.
- Report adverse events (AE/SAE) per agreements.
- Maintain high professional standards and stay abreast of advancements.
- Commit to ongoing learning and development.
- Perform other responsibilities as required by SDM SHN.
- Registered Nurse with current, unrestricted license in the province of employment.
- BScN preferred; experience and education considered.
- Proficiency in reimbursement landscape and completion of Special Authorization and Appeal letters.
- Experience with Patient Support Programs (PSP) a plus.
- Strong customer focus and ability to deliver PSP value proposition.
- Bilingual (French & English) fluent is an asset.
- Five or more years of direct clinical experience.
- Excellent communication and telephony skills.
- Demonstrated leadership experience and ability to work under pressure.
- Organized, efficient case‑management approach.
- Strong assessment, communication, critical thinking and decision‑making skills.
- Understanding of SDM SHN policies, legal considerations and professional ethics.
- Computer skills including Microsoft 365.
- Adaptability to a changing environment.
- Commitment to quality client‑centered service delivery.
- Travel required up to 60% with occasional overnight stays.
- Valid driver’s license and reliable vehicle.
Full time, Regular.
Salary range: $80,000 – $110,000 per year.
