You will engage in comprehensive patient care utilizing your full NP practice scope, navigating patients' journey from health promotion to complex disease management. Collaborate with other healthcare professionals and apply EMR systems for effective program development that leads to better community health outcomes.
Key Responsibilities:
• Manage health consultations and patient medication
• Conduct assessments for lifestyle and chronic disease programs
• Develop patient-centered care plans through collaboration
• Keep accurate EMR records and generate reports
• Organize and lead educational sessions
Requirements:
• Valid Extended Class registration with CNO
• Primary Care Nurse Practitioner Certificate required
• At least 2 years of experience in primary care
• Strong assessment and decision-making skills
• Flexibility to work in various locations
