RN Case Manager Care Coordinator

Bayshore Medical Personnel is excited to announce a full-time contract opportunity for a RN Case Manager (Care Coordinator) to join our team in Winnipeg, MB ! We're looking for dedicated professionals with at least 3 years of experience to provide service to our client’s members who suffer from protracted and complex health issues.

RN Case Manager’s work as a member of a multidisciplinary primary health care team in a primary care/clinic setting. Their primary responsibility is to complete detailed case management assessments, develop comprehensive care plans that reflect individual health care needs, and monitor clients and make suggestions for modifications as necessary.

Successful candidates will possess 3 full-time equivalent years of clinical experience within the last 4 years in nursing practice, including previous case management experience.

• Baccalaureate Degree in Nursing from a recognized university
• Certificate of Basic Life Support (BLS) for Healthcare Provider or CPR/AED Level
  
  C. Annual recertification is required
• Current license to practice from a provincial or territorial College of Nurses in the province of practice
