Position Summary
The Licensed Practical Nurse provides direct nursing services to individuals with physical and intellectual disabilities that focus on health promotion, restoration, maintenance, and prevention of illness and disability and palliative care. The Nurse is responsible for providing total patient care but may delegate some aspects of care to other health care team members in a staff mix model – e.g. Resident Assistants.
In addition to providing direct practical nursing services, the Nurse functions as a key member of the professional health care team and is responsible to provide day-to-day instruction and feedback to the Resident Assistants assigned to their care team.
Qualifications
Educational:
- Graduate of an approved LPN educational program
Professional:
- Holds current active practicing membership with the CLPNM
- Responsible to maintain competency in CPR/Code Blue
- Practices within the professional, ethical, legal and legislative parameters of the Licensed Practical Nursing Act, CLPNM Standards of Practice and Canadian Nurses Association Code of Ethics
- Adheres to PHIA guidelines of confidentiality of personal health information and respects the confidentiality of personal staff information
- Assumes responsibility for remaining current in their area of professional nursing practice and seeks opportunities for professional growth and learning
- Recognizes limitations of own abilities and seeks direction and consultation as necessary and accepts accountability for own actions and decisions
- Supports and participates in research related to people supported and the development of evidence based nursing practice
- Models professional behaviour with people supported, families and other members of the health care team
- Ability to make effective use of computer technology necessary for effective care planning, documentation and communication (e.g. MDS Software, Microsoft Office software)
Personal:
- Demonstrates ability to function effectively as a team leader and team member and possess the emotional stability necessary to serve as a model regarding cooperative teamwork, conflict resolution and quality improvement
- Demonstrates strong leadership and organization skills including the principles of delegation and supervision of care delivery by other members of the health care team (i.e. Resident Assistants)
- Demonstrates effective written and verbal communication skills
- Demonstrates knowledge of and support for the mission and philosophy of the St.Amant and the goals and objectives of Health & Transition Services.
Duties and Responsibilities
Client Care Management:
LPN's provide comprehensive direct nursing services to assigned people supported, in keeping with the scope of Nursing Service's mandate and the full scope of clinical practice identified by the CLPNM, in the following areas:
Assessment duties include:
- Assess people supported using a variety of resources and focusing on biological, emotional, socio-cultural, spiritual and quality of life factors
- Use knowledge from practical nursing and data from other members of the health care team to analyze and interpret the data necessary to identify potential and actual health needs/problems
- Perform ongoing assessment to identify changes in person supported health status
