The Primary Care Nurse will ensure accessible healthcare for individuals and families while working within the Extended Hours Primary Care Clinic. You will coordinate health plans, manage chronic diseases, and emphasize illness prevention. This role requires engagement with clients to foster self-management and optimal health outcomes through individualized care and education.
Key Responsibilities:
• Provide comprehensive healthcare across all age groups
• Develop and evaluate client-centric health plans
• Collaborate with interdisciplinary teams for coordinated care
• Support chronic disease management and prevention strategies
• Navigate clients to appropriate health resources
Requirements:
• Minimum two years in primary/community health nursing
• Graduate of a Registered Nursing education program
• Maintain registration with CRNM
• Certification in Diabetes or Asthma Education preferred
• Competently engage with Indigenous customs and languages
