Position Summary and Scope of Service

Under the general supervision of the Director, Health and Transition Services and in collaboration with the Medical Director, the Nurse Practitioner (NP) works within an interdisciplinary team in related roles as a practitioner, educator and counsellor. Operating within the mission, vision and values of St.Amant, the NP is a leader in integrated health care delivery, focusing on health promotion, disease and injury prevention, clinical interventions and treatments and management of simple stable and acute chronic disease on an individual, family and community basis.

The NP improves access to services for people who require health status assessments, periodic monitoring and management, and stable chronic disease surveillance and management. The NP works with individuals, families and groups to promote health and education, applying skills in consultation, collaboration, systems management and providing effective care. May be required to provide primary nursing care in a variety of clinical settings, including the Health and Transition Service program site or in a community setting at a persons' home.

Education considered an asset includes, but is not limited to:

  • College of Registered Nurses of Manitoba Standards of Practice for Registered Nurses on the Extended Practice Register
  • Canadian Nurses Association Code of Ethics for Registered Nurses
  • Scope of practice as documented in the Regulated Health Professionals Act
  • Principles of collaborative practice
  • Roles and responsibilities of members of the primary health care team
Qualifications and Duties
  • Master of Nursing, Nurse Practitioner Stream or equivalent educational preparation as approved by the College of Registered Nurses of Manitoba (CRNM)
  • Member in good standing with the CRNM and a Registered Nurse on the Extended Practice Register
  • Five (5) years' experience within nursing practice supporting people across the life span
  • Valid 5F (full) Driver's License and access to a reliable vehicle
  • Experience working with people who have a diagnosis of developmental disability, autism and/or acquired brain injury an asset
  • Cardiopulmonary training at the Health Care Level
  • Knowledge of health promotion, community development and program evaluation concepts
  • Demonstrate leadership ability, organizational, interpersonal, critical thinking/problem-solving and decision-making skills
  • Good verbal and written communication skills
  • Knowledge of the principles of adult education and interdisciplinary teamwork and ability to use group process techniques
  • Demonstrate and practice daily the nursing values and ethical responsibilities as set forth by the CRNM
  • Accept the possibility of being exposed to infectious diseases, noise and allergens
  • Be able to work a variable schedule to participate in meetings and activities outside normal working hours
  • Performs comprehensive and focused health assessments, synthesizes health assessment information, identify concerns and risks, as well as normal and abnormal conditions and makes differential diagnosis
  • Orders appropriate screening and diagnostic tests and interprets test results based on sound clinical reasoning
  • Demonstrates sound clinical judgment and sets criteria to assess the initial and subsequent outcomes of the health care plan, including consultation, referral and review of the plan based on the assessment results
