Position: Mental Health Nurse with PACt Team
Deliver essential case management and support services as a Nurse 3 within the PACt team specializing in severe mental illness care. Join Shared Health in Manitoba's community-focused approach to treatment.

As a Nurse 3, you will operate under the supervision of the PACT Team Leader to provide comprehensive care management to individuals with severe mental illnesses. Your responsibilities will involve coordinating medical services, medication management, and providing psychosocial rehabilitation. You will support clients dealing with psychiatric illnesses and collaborate within an interdisciplinary team to ensure quality care.

Key Responsibilities:

• Provide case management services for community participants

• Coordinate medical services and manage medications

• Assess and develop treatment plans collaboratively

• Educate individuals and families on medication use

• Offer crisis support and after-hours on-call assistance

Requirements:

• Minimum four years of community nursing experience

• Registered Nursing degree or equivalent required

• Current licensure with Manitoba nursing colleges

• Knowledge of mental health treatment protocols

• Valid driver's license and vehicle for community work
