This role requires a minimum of four years of experience in community services working with individuals affected by serious psychiatric conditions. You will provide essential case management, act as a liaison among patients and community partners, and coordinate medical services and medication management. Your contributions will significantly impact the lives of program participants by offering tailored treatment and support.
Key Responsibilities:
• Provide case management services in community settings
• Act as a liaison among participants and support systems
• Coordinate medical services and manage medications
• Assess and develop treatment and rehabilitation plans
• Provide crisis support and 24-hour assistance
Requirements:
• Nursing degree (BScMH, BSc.PN, BN, RPN, RN)
• Registered with Manitoba Nursing and Psychiatric Colleges
• 4+ years community mental health experience
• Knowledge of relevant Mental Health legislation
• Valid driver’s license and vehicle
