Community Nurse in PACT Program

This role requires a minimum of four years of experience in community services working with individuals affected by serious psychiatric conditions. You will provide essential case management, act as a liaison among patients and community partners, and coordinate medical services and medication management. Key Responsibilities:

• Provide case management services in community settings

• Act as a liaison among participants and support systems

• Coordinate medical services and manage medications

• Assess and develop treatment and rehabilitation plans

• Provide crisis support and 24-hour assistance

Requirements:

• Nursing degree (BScMH, BSc.PN, BN, RPN, RN)

• Registered with Manitoba Nursing and Psychiatric Colleges

• 4+ years community mental health experience

• Knowledge of relevant Mental Health legislation

• Valid driver’s license and vehicle
