Position Overview
Consistent with the principles of primary healthcare, this role provides access to first‑level basic healthcare within the scope of nursing practice for individuals, families, groups, and communities. As a member of an interdisciplinary community area team, the Primary Care Nurse delivers comprehensive care with an emphasis on healthy living, illness prevention, health education, chronic disease management, clinical intervention, and palliation. The nurse develops and implements health plans with clients, evaluates outcomes, coordinates ongoing services, and links clients to community resources.
Responsibilities- Actualize the mission, vision, and values of the Winnipeg Regional Health Authority.
- Apply the principles of Primary Health Care as outlined by the World Health Organization Declaration of Alma Ata.
- Synthesize knowledge from the Canadian Taskforce on Preventative Health Care and professional nursing theory to promote the health of individuals and families.
- Partner with family physicians to focus on a shared client population.
- Provide care to individuals across the life span within the context of their environment.
- Promote wellness and prevent disease and disability.
- Manage chronic conditions within a primary‑care setting.
- Support clients’ self‑determination and health‑management abilities.
- Maintain client‑centered care with an interdisciplinary team approach.
- Navigate clients to appropriate referrals and services.
- Adopt a long‑term, family‑centred nursing philosophy to ensure continuity of care.
- Offer harm‑reduction and trauma‑informed care.
- Minimum two years of directly related experience in primary healthcare, community health nursing, northern nursing, or ambulatory care nursing with an emphasis on primary and secondary prevention, intervention, and palliation.
- Experience with mental health counselling is beneficial.
- A satisfactory employment record is required.
- Graduate of an approved Registered Nursing education program.
- Certification in Diabetes or Asthma Education (or attainment within 2–3 years of employment) is preferred.
- Maintain and provide proof of registration with the College of Registered Nurses of Manitoba (CRNM).
- Active participation in professional associations such as the Association of Regulated Nurses of Manitoba is preferred.
- Cardiopulmonary Resuscitation (CPR) training in accordance with employer policy.
- Possession of a valid Manitoba Class 5 driver’s license.
- Scope of practice as defined by the Regulated Health Professions Act (RHPA).
- Proficiency with computers, word processing, spreadsheets, and databases.
- Physical examination and nursing intervention skills.
- Commitment to interdisciplinary teamwork.
- In-depth knowledge of primary care and community health.
- Effective oral and written communication skills.
- Preference for those competent in an Indigenous language or familiar with Indigenous customs, traditions, and values.
- Physically capable of performing job duties, including lifting equipment, assisting clients, and arranging group teaching activities.
- Ability to provide care in various settings such as a clinic, community sites, clients’ homes, or private physician practices.
- Access to suitable transportation for Primary Care Nurse duties.
This term position may end earlier as outlined in your collective agreement.
This position requires a current satisfactory Criminal Records Check, including Vulnerable Sector Search, Child Abuse Registry Check, and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if obtained no more than six months prior to the start of employment.
Interviewed candidates may be called to participate in a skills assessment.
