This role, functioning under the Home Care Nursing unit, is designed for Registered Nurses with a desire to make an impact in community health. You will work with clients exhibiting complex health needs while demonstrating effective communication and leadership within the healthcare team. This position involves direct client care, coordination of discharge, and service delivery in home and community environments.
Key Responsibilities:
• Provide nursing care in homes, schools, and businesses
• Administer prescribed treatments and medications
• Lead care coordination among healthcare professionals
• Monitor and document client progress and needs
• Facilitate client discharge and follow-up care
Requirements:
• Graduate from an approved Registered Nursing program
• Minimum two years relevant nursing experience
• Active CRNM registration and CPR certification
• Valid Class 5 driver's license is required
• Additional certifications in wound care preferred
