Nurse Practitioner in Home Care

Become a Nurse Practitioner focusing on home assessments and patient education. This role emphasizes client collaboration, care management, and medication review to enhance member health outcomes.

We are looking for an experienced Nurse Practitioner in home care to support Alignment members. Your expertise will involve conducting in-home assessments, reviewing medications, and educating patients on care options. Collaboration with the Regional Medical Officer and other healthcare team members is vital to creating effective care plans tailored to patient needs.

Key Responsibilities:

• Conduct in-home assessments and health screenings

• Provide patient education and medication reviews

• Identify diagnoses for care management

• Collaborate with PCPs, specialists, and stakeholders

• Lead clinical team including nurses and social workers

Requirements:

• Master’s degree from accredited NP or PA program

• Preferred one year clinical or home care experience

• Active Nurse Practitioner or Physician Assistant license

• Strong communication, problem-solving, and time management skills

• Valid Driver License and current automobile insurance
