At Welbe Health, our Home Health RN position centers on supporting elderly participants through personal, in-home care. You will work alongside an interdisciplinary team to create compassionate care plans, ensuring comfortable access to health services. This unique role allows you to skip OASIS assessments, emphasizing patient care instead, while maintaining a flexible schedule to achieve work-life balance.
Key Responsibilities:
• Conduct comprehensive home health nursing assessments
• Collaborate in IDT meetings to develop care plans
• Deliver and document nursing interventions and care
• Educate participants and families on self-care techniques
• Monitor and manage scheduling with Home Health Manager
Requirements:
• RN License from an accredited nursing program
• Minimum one year of nursing experience
• Experience with elderly or frail populations preferred
• Reliable transportation for home visits
• CPR and First Aid certification within 180 days
