As a vital team member under the Director of Primary Care, you will conduct health assessments, support chronic disease education, and triage urgent cases. This position involves full-time hours of 77.50 biweekly, fostering a rewarding environment committed to health promotion. Your expertise will empower marginalized communities through targeted healthcare services.

Key Responsibilities:

• Perform sexual health examinations and comprehensive assessments

• Educate patients on chronic disease management

• Provide urgent triage and supportive care

• Maintain ongoing case management for high-risk populations

• Work collaboratively with healthcare professionals to enhance care

Requirements:

• Baccalaureate in Nursing or equivalent clinical experience

• Active registration with the College of Registered Nurses of Manitoba

• Current Cardiopulmonary Resuscitation certification

• Asset: RN Authorized Prescriber for reproductive health

• A dedication to serving diverse community needs
