Registered Nurse - Palliative Care Specialist

In this position, you will support patients at Hilltop House Long Term Care Facility who are dealing with severe, life-limiting illnesses. Through nursing assessments, symptom management, and collaborative care plans, you will play a vital role in achieving goals of care and easing emotional and physical challenges faced by patients and their families. Your focus on best practices will enrich the lives of those you serve.

Key Responsibilities:

• Deliver nursing care to complex palliative patients

• Collaborate in interdisciplinary care team approaches

• Assess and manage patient care needs effectively

• Facilitate goals of care conversations with patients

• Document patient care in accordance with standards

Requirements:

• Active RN registration with BCCNM

• Two years relevant medical-surgical experience

• Palliative care training preferred

• Demonstrated assessment skills for patient care

• Ability to provide emotional support to families
