Sentrex Health Solutions is seeking a Field Nurse Case Manager to support patient services across Western Canada. This hybrid, field-based role requires clinic visits, patient enrolment support, and collaboration with physicians to optimize therapy access. Strong clinical acumen and exceptional communication are essential.
You will travel within the assigned region up to 75%, maintaining patient confidentiality and delivering comprehensive case management with adherence to program protocols.
