Key Responsibilities:
• Offer case management services for program participants.
• Coordinate medical services and medication delivery.
• Develop and assess individualized treatment plans.
• Facilitate connections with community partners.
• Provide after-hours crisis support as required.
Requirements:
• At least four years of relevant community experience.
• Nursing degree and registration with Manitoba colleges.
• Familiarity with psychiatric care legislations.
• Ability to work with diverse populations effectively.
• Valid driver’s license and vehicle are mandatory.
