Case Manager for Newborns

Position: Full-Time Case Manager for Newborns
Make a difference as a Full-Time Case Manager supporting mothers and newborns. This position emphasizes individualized care plans, coordination of services, and community connections.
The successful candidate will work weekdays from 8am to 4pm, providing support to families through comprehensive assessments and personalized case planning. The role requires collaboration with various community partners, child welfare, and stakeholders to foster parenting capacity and stability.

Key Responsibilities:

• Conduct comprehensive assessments for mothers and newborns

• Develop individualized case plans for family preservation

• Provide coaching and advocacy for residents

• Assist with navigating housing and healthcare systems

• Coordinate wraparound services including mental health supports
Requirements:

• Diploma/degree in Social Work or related field

• Minimum 3-5 years of case management experience

• Strong understanding of Indigenous culture and issues

• Ability to work in a trauma-informed environment

• Valid driver’s license and access to a car
