Scheduled Monday to Friday, 8am to 4pm, you'll provide culturally safe support while developing individualized care plans. Collaborating with community resources, you will advocate for mothers navigating complex systems and promote family stability.
Key Responsibilities:
• Conduct intake and ongoing assessments for families
• Create tailored case plans focusing on wellness
• Advocate and coach mothers through challenges
• Coordinate access to crucial community supports
• Liaise with healthcare and child welfare partners
Requirements:
• Diploma or degree in Social Work or related field
• 3-5 years of relevant case management experience
• Knowledge of Indigenous history and child welfare
• Strong crisis intervention and documentation skills
• Current First Aid/CPR and a valid driver's license
