Join Blue Thunderbird Family Care Inc. as a dedicated Case Manager in Winnipeg. Support mothers and newborns with tailored care plans while advocating for community resources and wellness initiatives.
This full-time position requires a compassionate individual eager to improve parenting capacity and family stability. You will conduct comprehensive assessments, develop individualized case plans, and coordinate critical services in collaboration with families, community partners, and child welfare systems. Your expertise will make a direct impact on the lives of residents, ensuring culturally safe and trauma-informed support.
Key Responsibilities:
• Conduct intake and ongoing assessments for mothers and newborns
• Develop personalized care plans focused on family wellness
• Provide one-on-one coaching and advocacy
• Assist mothers with housing and income navigations
• Coordinate wraparound prenatal and postnatal services
Requirements:
• Diploma/degree in Social Work or related field
• 3-5 years of relevant experience in case management
• Understanding of Indigenous culture and history
• Crisis intervention and de-escalation skills
• Valid Class 5 driver’s license and First Aid/CPR certification
