Position Summary:
The Case Manager plays a key role in supporting mothers and newborns living in the home. This position works directly with residents to develop and implement individualized care plans that promote parenting capacity, stability, and connection to community supports. The Case Manager provides advocacy, coordination of services, and culturally safe support while working collaboratively with families, Elders, community resources, and child welfare partners.
- Conduct comprehensive intake and ongoing assessments for mothers and newborns.
- Develop individualized case plans focused on family preservation, parenting support, and wellness.
- Provide one-on-one support, coaching, and advocacy for residents.
- Assist mothers in navigating systems including housing, income supports, healthcare, and parenting resources.
- Support mothers in meeting requirements related to child welfare involvement when applicable.
- Coordinate wraparound services such as prenatal and postnatal care, mental health and addictions supports, parenting education, housing stabilization, and cultural supports.
- Liaise with community partners, health providers, and child welfare workers.
- Ensure services are delivered in a trauma-informed, culturally safe, and strength-based manner.
- Maintain accurate case notes and documentation.
- Advocate for residents’ rights and family preservation.
- Participate in case conferences and team meetings.
- Be legally entitled to work in Canada.
- Produce satisfactory background checks.
- Have a valid Class 5 driver’s license and access to a car.
- Be physically capable of performing all childcare duties, including the ability to lift 50 lbs.
- Have current First Aid/CPR Level C Certification.
- Diploma or degree in Social Work, Human Services, Indigenous Studies, or related field (or equivalent lived/work experience).
- Minimum 3–5 years of experience in case management, preferably with mothers, infants, or family preservation programs.
- Strong understanding of Indigenous history, culture and the impacts of colonization, intergenerational trauma and the child welfare system.
- Experience working in trauma-informed, harm reduction, and culturally safe environments.
- Compassionate and non-judgmental approach.
- Strong advocacy and problem-solving skills.
- Crisis intervention and de-escalation abilities.
- Excellent documentation and organizational skills.
- Ability to work collaboratively in a multidisciplinary team.
