You will provide assessment and case management services for oncology patients and families, navigating barriers to care. Collaborating with Indigenous communities and organizations, your efforts will contribute to improving health equity and the oncology experience for patients. This position is permanent and part-time, allowing you to make a significant impact on the lives of those in need.
Key Responsibilities:
• Provide culturally safe case management services to patients
• Assess social and systemic factors affecting care access
• Collaborate with internal and external partners for support
• Advocate for Indigenous patients within cancer care systems
• Contribute to initiatives promoting health equity in oncology
Requirements:
• Bachelor or Master of Social Work degree
• 2 years of related social work experience
• Experience with Indigenous communities in Manitoba
• Commitment to trauma-informed care principles
• Licensure as a Social Worker in Manitoba
