Duties
  • The Community Service Worker (CSW) for Community Living dis ABILITY Services (CLdS) and Children's dis ABILITY Services (CdS) participates in, and often leads, a Service Coordination team that may include Health, Education and Early Childhood Learning, child welfare agencies, private non-profit agencies, Justice, Employment and Income Assistance (EIA), and Manitoba Supports for Persons with Disabilities (MSPD).
  • Acts as a liaison between families, advocates, other government and community-based agencies, and the individuals served, using a family-centred/person-centred approach to deliver discipline-based services and intervention strategies.
  • Coordinates and delivers protection services, support services, crisis management, and the Substitute Decision-Making process as outlined in The Adults Living with an Intellectual Disability Act.
  • Assesses, plans, implements, and monitors services for children, families, or vulnerable individuals; collaborates with professionals; advocates for program clients; plans transitions to childcare, schooling, and alternate placements; coordinates day activity programming; counsels program clients; and represents CLdS/CdS to stakeholders, the community, and families.
  • Addresses issues arising from domestic violence, trauma, disability, abuse, mental and emotional challenges, loss, addiction, homelessness, and family breakdown; develops supports and networks to enable independence and improved functioning.
  • Positively impacts the lives of families, children, and clients, enhancing their experiences moving forward.
Qualifications

Essential

  • Post-secondary education in the social services field (Bachelor of Social Work preferred). Foreign credentials require official assessment; if educated outside Canada, equivalency must be demonstrated.
  • Case management experience in an integrated services environment including brief counselling, assessments, service planning and coordination, resource development and follow-up.
  • Experience working with children with disabilities or vulnerable adults and their families; ability to tailor supports to individual needs.
  • Experience conducting risk assessments with participants and families.
  • Experience with crisis intervention and protection services.
  • Effective verbal and strong written communication skills.
  • Strong interpersonal skills with the ability to interact in a respectful, non-judgmental manner to resolve conflicts with diverse stakeholders.
  • Excellent time-management and organizational skills; ability to manage high-volume caseloads in a fast-paced environment.
  • Ability to work independently and take initiative within a multi-disciplinary team.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook) or similar software.

Desired

  • Ability to read, write, and verbally communicate in French.
