Position: Case Worker for Supportive Housing Program
Become a Case Worker in the Supportive Housing Program, assisting individuals to transition and maintain permanent housing. This full-time role emphasizes trauma-informed care and client-centered support.
As a Supportive Housing Case Worker reporting to the Manager of Residential Support Services, you will manage casework for participants aiming to secure stable housing. Through collaboration, advocacy, and dedicated support, you will assist clients in navigating complex situations and accessing essential resources.

Your role is pivotal in helping participants achieve their housing goals.

Key Responsibilities:

• Complete intake assessments and follow-up case management plans

• Help clients access Employment and Income Assistance

• Facilitate necessary shopping and supplies for new housing

• Conduct home visits to monitor participant progress

• Maintain comprehensive participant files and records
Requirements:

• Driver’s license and access to transport mandatory

• Knowledge of community resources and housing assistance

• Experience providing crisis intervention and case management

• Strong communication and interpersonal skills

• Attention to detail and proficiency in record keeping
