Case Manager for Sacred Care Homes

Join the Sacred Care team as a Case Manager, providing vital support across foster homes within Winnipeg. This role focuses on communication, case planning, and holistic care management.

As a Sacred Care Case Manager, you will collaborate with foster parents, youth, and support teams to ensure comprehensive care and case management services. Your responsibilities will include conducting monthly meetings, maintaining accurate documentation, and identifying training needs for staff. Your insights will directly impact the quality of life for the families involved in the Sacred Care program.

Key Responsibilities:

• Establish relationships with foster parents and youth via home visits

• Conduct monthly case planning meetings with assigned collaterals

• Complete Foster Care Licensing documentation accurately

• Collect, review, and distribute monthly reports for homes

• Communicate observed training needs to the Program Manager

Requirements:

• BSW degree or equivalent experience in the field

• Knowledge of Child and Family Services sector

• Excellent conflict resolution skills and team collaboration

• Valid Manitoba driver’s license and reliable vehicle

• Ability to handle up to 50lbs and work with confidentiality
