POSTING # 26-01-01 POSITION TYPE TERM (To March 2028), FULL-TIME
Background: The Ma Mawi Wi Chi Itata Centre is currently seeking an individual for the position of Care Planner. The incumbent will work as part of the Wi Che Win team to provide holistic support services to program participants, including youth experiencing homelessness. Activities will include individual support services, home visits, and assistance with individualized plans.
Brief overview of some duties: In consultation with the team, provide support and accompany participants as they transition from homelessness to stable housing. Assist with acquiring and maintaining homes, accessing emergency housing if required, and developing knowledge and skills for successful tenancy. Mandated responsibilities include responding immediately to crises, assessing participants’ needs, and delivering services in a timely, professional, and effective manner.
Qualifications:
- Youth Care Diploma or an acceptable equivalent and related experience.
- Experience in care planning and implementing healing plans with participants.
- Knowledge of crisis intervention strategies.
- Experience building and maintaining productive relationships with Indigenous participants.
- Ability to liaise and consult with partner agencies regarding specific service recipients or general concerns.
- Strong teamwork and cooperative work skills.
- Ability to perform under stressful situations.
- Excellent oral and written communication skills.
- Understanding of Indigenous values.
- Must possess a valid Manitoba Driver’s
