Join the Ma Mawi Wi Chi Itata Centre as a Care Planner focusing on holistic support for youth. Help participants transition from homelessness to stable housing through individualized plans and professional services.
As a full-time Care Planner, you'll be part of the Wi Che Win team, directly impacting the lives of youth experiencing homelessness.
Your role encompasses individual support services, home visits, and crisis response. Collaborate with team members and external agencies to ensure effective service delivery and contribute to achieving successful tenancy for participants.
Key Responsibilities:
• Provide individual support services and home visits
• Assist with housing transitions and emergency accommodations
• Respond promptly to crises and assess participant needs
• Develop personalized plans for successful living
• Collaborate with partner agencies for better outcomes
Requirements:
• Youth Care Diploma or equivalent experience
• Background in care planning and crisis intervention
• Strong relationship-building skills with Indigenous participants
• Valid Manitoba Driver’s License required
• Excellent communication abilities
