Sacred Care Case Manager

Position: Sacred Care Case Manager Role
Empower the Sacred Care homes as a Case Manager, focusing on case management and support. This position requires travel within Winnipeg, strengthening connections among families and programs.

In this Case Manager role with Sacred Care, you will engage with foster parents, youth, and the support team to deliver holistic and comprehensive case management services. Responsibilities include managing case planning, ensuring compliance with licensing standards, and fostering ongoing communication to address training and support needs effectively. Your efforts will enhance service delivery for families in care.

Key Responsibilities:

• Build positive relationships through regular home visits and communication

• Conduct monthly meetings to plan and strategize care

• Maintain comprehensive service documentation and licensing compliance

• Distribute incident and monthly reports, addressing concerns

• Identify training needs and opportunities for staff development

Requirements:

• Bachelor of Social Work or similar qualification

• Familiarity with intergenerational trauma and colonization issues

• Strong communication and organization skills

• Clear criminal record and Child Abuse registry checks

• Valid Manitoba driver’s license and ability to lift 50lbs
