Background: The Ma Mawi Wi Chi Itata Centre is seeking an individual for the position of Care Planner. The incumbent will work as part of the Wi Che Win team to provide holistic support services to program participants, including the youth homelessness population. Services include individual support, home visits, and assistance with participants’ individualized plans.

Responsibilities

In consultation with the team, provide support and walk with participants as they transition from homelessness to stable housing. Assist participants in acquiring and maintaining a home, accessing emergency housing if needed, and providing the knowledge and skills required to become a good neighbor and successful tenant. Support participants’ individualized plans of action and their work on implementing goals. Respond to participants’ crises immediately, further assess their needs, and deliver services in a timely, professional, and effective manner.

Qualifications
  • Youth Care Diploma or acceptable equivalent and related experience.
  • Care planning and healing plan development and implementation with participants.
  • Working knowledge of crisis intervention strategies.
  • Demonstrated experience developing and maintaining an effective working relationship with Indigenous participants.
  • Liaison and consultation with collateral agencies about a specific service recipient or general service concerns.
  • Ability to work cooperatively with others and contribute as a team member.
  • Ability to work under stressful situations.
  • Strong oral and written communication skills.
  • Understanding of Indigenous values.
  • Valid Manitoba Driver’s Licence.
  • Must provide a current (within the last three months) Criminal Record with Vulnerable Sector Query, Child Abuse Registry Check, and be subject to Prior Contact Check.
